Connecticut Uniform Police Accident Report (Form PR1 Rev. 12/94)

Structure and general layout

Form PR1 is organised into two vertical halves, each corresponding to a vehicle and its operator. At the top of the form are spaces for GPS readings (latitude and longitude), the time and date of the accident, and whether the crash occurred day or night. Officers mark the accident severity (fatal, injury or property damage only) and note whether the crash occurred at an intersection, railroad grade crossing or private property. Operator and vehicle information

Each column of page 1 collects information about a motor‑vehicle operator or pedestrian involved in the crash. The officer records the operator’s name, address, city, state, zip code and phone number. Additional fields list the operator’s driver’s licence number and state, proper licence class (A/B/CDL etc.) and date of birth【405773302721902†screenshot】. The form captures whether the operator wore a safety belt and whether alcohol tests or sobriety tests were given, with checkboxes for yes, no and refused. Vehicle information includes the registration number, state, vehicle year and make, body type and vehicle identification number (VIN). The officer notes the carrier’s name for commercial vehicles and the carrier’s address. Checkboxes indicate whether the vehicle was towed, disabled or parked prior to the crash. For commercial vehicles, the officer identifies the gross vehicle weight rating (GVWR) and whether the vehicle was placarded for hazardous materials.

Traffic conditions and roadway environment

Form PR1 includes sections for documenting traffic control devices and roadway conditions. Officers specify whether the crash occurred at a location controlled by a traffic signal, stop sign, yield sign, no control device, police officer or flashing beacon. They also record road surface conditions (dry, wet, snow/ice, sand/mud, oil) and lighting conditions (daylight, dawn/dusk, dark with street lights, dark without street lights). The form captures weather conditions such as clear, cloudy, rain, snow/sleet and fog/mist. To describe the collision dynamics, the officer marks the manner of collision (head‑on, rear‑end, angle, sideswipe, backing, other) and the first harmful event (collision with motor vehicle, pedestrian, bicycle, fixed object, overturn, other non‑collision). The form also records the posted speed limit and estimated travel speed for each vehicle.

Occupant and injury data

At the bottom of page 1 is a table labelled “Name and Address of Each Involved Person”【405773302721902†screenshot】. Rows correspond to occupants and pedestrians, while columns capture: * Traffic unit number – linking the person to a specific vehicle or pedestrian entry. * Involved person’s name and address. * Date of birth and sex. * Operator licence number for drivers. * Nature of injury – fatal, incapacitating injury, non‑incapacitating injury, possible injury or no injury. * Location in vehicle – driver, front seat passenger, rear seat passenger, cargo area, motorcycle operator/passenger or pedestrian. * Ejection and restraint use – officers mark whether the occupant was ejected and the type of restraint used. * Transported to – the hospital or facility where the injured person was taken. The form provides 10 rows for involved persons; additional pages are used for larger crashes. This table ensures that all occupants, regardless of severity, are recorded in a uniform manner.

Codes and narrative

Form PR1 uses numeric codes to capture specific details such as contributing circumstances (driver behaviour, road conditions, vehicle defects), vehicle actions prior to impact (straight, turning, backing, parked), trafficway character (divided, undivided, one‑way), traffic flow (two‑way, one‑way), and road alignment (straight, curve, grade). Officers consult an accompanying code sheet to enter the appropriate numbers. Although the first page emphasises structured data, subsequent pages of the Connecticut report include a diagram area and a narrative section. The diagram allows the officer to sketch the roadway configuration, vehicle paths and impact points, while the narrative provides a free‑form description of the crash sequence, contributing factors and any unusual circumstances.
