How to Request a Police Report in New York

In New York State, police reports can be requested from the New York State Police for incidents on state highways, or from local police departments including the NYPD for New York City incidents. The DMV requires Form MV-104 for all reportable accidents. Online requests are available through the NYS DMV website or individual police department portals.
