How to Request a Police Report in New York
In New York State, police reports can be requested from the New York State Police for incidents on state highways, or from local police departments including the NYPD for New York City incidents. The DMV requires Form MV-104 for all reportable accidents. Online requests are available through the NYS DMV website or individual police department portals. You'll need the incident date, location, and report number if available.
Required Documents
New York requires a valid driver's license or state ID, the police report number or accident date/location, names of involved parties, and appropriate fees ($10-$30 depending on agency). For NYPD reports, you may need to visit the precinct where the report was filed. Insurance companies can request reports with a notarized letter of authorization from their insured.
